My Possibilities (“My Possibilities,” “MP,” “we,” “us,” or “our”) respects the privacy of the individuals and families we serve, our donors, volunteers, employees, applicants, community partners, website visitors, and others who interact with us.

This Privacy Policy explains how we collect, use, retain, protect, and disclose personal information through:

  • Online forms and portals that link to this Privacy Policy;
  • Admissions and program inquiries;
  • Donations, payments, and fundraising activities;
  • Volunteer and employment inquiries;
  • Events and community-engagement activities;

  • Relationship to a participant, applicant, donor, volunteer, or employee;

  • Donation history and fundraising preferences;
  • Payment and transaction information;

Admissions and Program Information

When an individual applies for or participates in a My Possibilities program, we may collect information necessary to evaluate the application, provide appropriate services, administer the program, communicate with the individual or family, and satisfy legal, contractual, or funding requirements.

Depending on the program and circumstances, this information may include:

  • Disability-related information;
  • Medical or health-related information;
  • Medication or support information;
  • Educational records;
  • Behavioral or developmental information;
  • Assessments and evaluations;
  • Guardianship or authorized-representative information;
  • Insurance or benefits information;
  • Medicaid-waiver or other funding information;
  • Attendance and participation records;
  • Transportation information; and
  • Information contained in supporting documentation.

This information may be subject to separate program notices, authorizations, releases, and confidentiality requirements.

Donation and Payment Information

When you make a donation or payment, we or our payment service providers may collect:

Payment-card information is generally collected and processed by third-party payment processors. My Possibilities generally receives transaction details and confirmation but does not intend to store complete payment-card numbers or card security codes.

Volunteer and Background-Check Information

When you apply to volunteer, we may collect information concerning your identity, contact information, availability, experience, interests, references, and suitability for a volunteer role.

Certain volunteer roles may require a confidentiality agreement, screening, or criminal background check. Background-check information may be collected directly by My Possibilities or by an authorized screening provider and will be handled in accordance with applicable law.

Employment Applicant Information

Employment applications may include:

  • Contact information;
  • Employment history;
  • Education;
  • Professional qualifications;
  • References;
  • Resume or curriculum vitae;
  • Work authorization information;
  • Voluntary demographic information;
  • Accommodation requests; and
  • Other information provided during the recruitment process.

Employment applications submitted through a third-party recruiting platform are also governed by that platform’s privacy policy.

Communications and Text-Message Information

When you communicate with My Possibilities by telephone, email, webform, SMS, or MMS, we may collect:

  • The date and time of the communication;
  • The content of your message;
  • Attachments or images you send;

5. Text Messaging and Mobile Information

My Possibilities may use SMS or MMS text messaging for conversational, informational, operational, program-related, event-related, fundraising, or promotional communications when appropriate consent has been obtained.

Message frequency may vary. Message and data rates may apply.

You may opt out of text messages at any time by replying STOP. You may reply HELP for assistance. We will also honor other reasonable requests that clearly communicate that you no longer wish to receive text messages.

Opting out of text messages does not necessarily prevent My Possibilities from contacting you by telephone, email, postal mail, or another permitted method.

No mobile opt-in information or text-message consent will be shared with third parties or affiliates for marketing or promotional purposes.

We may disclose mobile numbers and related information to communications providers and other service providers solely as necessary to send messages, maintain our communications systems, document consent, process opt-out requests, prevent fraud, or provide services on our behalf. Those providers are not authorized to use mobile opt-in information for their own marketing purposes.

Consent to receive promotional or fundraising text messages is not a condition of enrollment, participation in a program, receipt of services, making a donation, or making a purchase.

Sensitive Information and Text Messages

Text messaging is not intended for emergencies or for transmitting highly sensitive information. Do not send the following information by text message:

  • Social Security numbers;
  • Complete payment-card information;

  • Detailed medical records;
  • Diagnostic reports;
  • Medication records;
  • Government identification numbers;
  • Information concerning abuse or an immediate safety threat; or
  • Other information that requires urgent, confidential, or secure handling.

For emergencies, call 911. For sensitive program or participant matters, contact the appropriate My Possibilities department using an approved secure method.

My Possibilities does not sell personal information for monetary consideration.

We do not knowingly disclose personal information to third parties for their own independent direct-marketing purposes without appropriate consent.

My Possibilities respects the privacy of its donors.

My Possibilities’ primary programs are designed for adults with Intellectual and Developmental Disabilities. Our general website is not directed to children under 13, and we do not knowingly use the general website to collect personal information directly from children under 13 without appropriate authorization.

A parent, guardian, or authorized representative who believes that a child has submitted information without appropriate permission should contact us so that we can review the matter.

 

15. Confidentiality of Participant Information

Information concerning My Possibilities applicants and participants may be confidential and may be protected by organizational policy, contract, consent requirements, or federal or state law.

Access to participant information is limited to individuals who have an appropriate operational, legal, safety, funding, or service-related need for the information.

This website Privacy Policy is not intended to authorize any disclosure that would otherwise be prohibited by an applicable confidentiality requirement.

 

16. Photographs, Video, and Testimonials

My Possibilities may take or use photographs, video, audio, stories, testimonials, or other media in connection with programs, events, fundraising, education, and community outreach.

When required, My Possibilities will seek an appropriate release or authorization before using identifiable participant media.

Consent to the use of media may be governed by a separate release and is not created solely by visiting our website.

When you interact with My Possibilities through a social-media platform, the platform may collect information about you and your interaction according to its own privacy policy.

Information posted publicly on social media may be visible to other users. Do not post confidential participant information, medical information, payment information, or other sensitive personal information on a public page.

We may update this Privacy Policy periodically to reflect changes in our practices, technology, services, vendors, or legal obligations.

When we make changes, we will post the updated Policy on our website and revise the “Last Updated” date. Material changes may also be communicated through another appropriate method.
